Semaglutide: The Reference Incretin
A selective GLP-1 receptor agonist with FDA-approved indications spanning type 2 diabetes, obesity management, cardiovascular risk reduction, and metabolic liver disease — the compound every newer incretin is measured against.
The short version
Semaglutide is a 31-amino-acid peptide engineered to mimic GLP-1, one of the gut hormones that tells your pancreas to release insulin after eating. Two key chemical changes give it a half-life of roughly one week instead of the two minutes that native GLP-1 survives: a substitution at position 8 blocks the enzyme that normally degrades it, and a fatty-acid chain attached at position 26 lets it hitch a ride on albumin in the bloodstream, protecting it from clearance.
In the STEP 1 trial — 1,961 adults with obesity, no diabetes — once-weekly subcutaneous semaglutide 2.4 mg produced a mean body-weight change of -14.9% at 68 weeks versus -2.4% with placebo [4]. That 12-percentage-point difference became the benchmark every subsequent incretin compound was compared against.
Two of the three is FDA-approved prescription medicines. Semaglutide is approved for type 2 diabetes, chronic weight management, reduction of cardiovascular events in adults with established cardiovascular disease and obesity, and (2025) metabolic liver disease. It is dispensed only by prescription; research-grade material sold outside this pathway is outside the approved evidence base. What it is
Semaglutide is a synthetic analogue of human GLP-1 sharing approximately 94% sequence homology with the native hormone. The two critical substitutions are: position 8 alanine replaced by alpha-aminoisobutyric acid (blocks DPP-4 cleavage), and position 34 lysine replaced by arginine. A C18 fatty di-acid side chain is then attached via a glutamic-acid/ADO spacer to the remaining lysine at position 26. This lipid moiety drives strong, reversible albumin binding — the structural basis for once-weekly dosing.
It is available as a once-weekly subcutaneous injection and as a once-daily oral tablet. The oral formulation relies on the absorption enhancer SNAC and has a bioavailability of only approximately 0.4-1%, making administration timing critical.
How it works
Semaglutide activates the GLP-1 receptor across multiple tissues simultaneously. At the pancreas, GLP-1R agonism potentiates glucose-dependent insulin secretion from beta cells and suppresses inappropriate glucagon from alpha cells. At the stomach, it slows gastric emptying, which flattens post-meal glucose spikes and contributes to a feeling of fullness. Neither of these effects causes hypoglycemia on their own because they require glucose to be present.
The weight-lowering mechanism is predominantly central. Rodent studies using pharmacological mapping showed that semaglutide reaches hypothalamic and brainstem appetite circuits — specifically the arcuate nucleus, the area postrema, and the parabrachial nucleus — where it activates anorexigenic (hunger-suppressing) neurons and inhibits hunger-promoting neurons, reducing food intake and modifying food preference without lowering energy expenditure [6].
What the research shows
Weight management — STEP 1. In 1,961 adults with overweight or obesity (no diabetes), once-weekly subcutaneous semaglutide 2.4 mg produced a mean body-weight change of -14.9% at week 68 versus -2.4% with placebo; the treatment difference was approximately 12.4 percentage points [4].
Cardiovascular outcomes — SELECT. In 17,604 adults with established cardiovascular disease and overweight/obesity but no diabetes, once-weekly semaglutide 2.4 mg reduced the primary composite of cardiovascular death, nonfatal MI, or nonfatal stroke versus placebo (HR 0.80; 95% CI 0.72-0.90; P<0.001) — a 20% relative risk reduction [3].
Kidney outcomes — FLOW. In 3,533 people with type 2 diabetes and chronic kidney disease, once-weekly semaglutide 1.0 mg reduced major kidney-disease events (kidney failure, ≥50% eGFR decline, or kidney/cardiovascular death) by 24% versus placebo (HR 0.76; 95% CI 0.66-0.88) [2].
Early cardiovascular trial — SUSTAIN-6. In 3,297 adults with type 2 diabetes at high cardiovascular risk, semaglutide 0.5 or 1.0 mg reduced the MACE composite (HR 0.74; 95% CI 0.58-0.95) — but also increased diabetic-retinopathy complications (HR 1.76; 95% CI 1.11-2.78), a finding attributed to the speed of glycemic correction rather than a direct retinal toxicity [7].
Head-to-head versus tirzepatide — SURMOUNT-5. In 751 adults with obesity (no diabetes), semaglutide 1.7 or 2.4 mg weekly was compared directly to tirzepatide at maximum tolerated dose. Semaglutide produced -13.7% mean weight change versus -20.2% for tirzepatide — an approximately 6.5 percentage-point disadvantage that was statistically significant (P<0.001) [1].
Central appetite mechanism. Pharmacological mapping in rodents characterized the neural pathways by which semaglutide reduces food intake: hypothalamic arcuate nucleus, area postrema, and brainstem parabrachial nucleus activation, with no decrease in energy expenditure [6].
Safety summary. A dedicated safety review describes a favorable overall risk/benefit profile, with predominantly mild-to-moderate transient GI effects (nausea in approximately one-third of patients), an established increase in biliary disease (cholelithiasis), and pancreatic/thyroid-cancer signals that lack definitive conclusion from low incidence [5].

Reported effects, cautions & safety
The community experience with semaglutide — drawing on patient reviews and post-market data — maps closely onto what clinical trials documented, with a few additions the trial reports underweight.
Frequently reported benefits: appetite suppression and the quieting of intrusive "food noise" is by far the most common benefit described, often appearing in the first week or two; weight loss through eating substantially less; markedly improved blood sugar and A1C readings in people using it for type 2 diabetes; reduced desire to drink alcohol (an unexpected secondary observation reported widely in patient communities). These are anecdotal, unverified reports, not clinical findings.
Frequently reported adverse effects: nausea is the single most reported side effect (approximately one-third of clinical-trial patients; peaking during dose escalation and after each dose increase); sulfur or egg-like burps linked to slowed gastric emptying; constipation and diarrhea, sometimes alternating; fatigue, especially in the first days after each injection; acid reflux and heartburn; headaches and dizziness (often hydration-related). These are anecdotal, unverified reports alongside the well-documented clinical-trial signals.
Cited safety cautions from the literature:
- GI intolerance during dose escalation is the primary adverse-effect profile and the leading cause of discontinuation [5].
- Personal or family history of medullary thyroid carcinoma or MEN-2 is a contraindication (boxed warning; rodent-derived, unconfirmed in humans) [5].
- Acute pancreatitis carries a class warning; the drug is stopped if suspected [5].
- Gallbladder and biliary disease (cholelithiasis) is increased versus placebo, attributed largely to the rate of weight loss [5].
- Pre-existing diabetic retinopathy with rapid glycemic correction: SUSTAIN-6 showed increased retinopathy complications (HR 1.76) in this context [7].
- Lean-mass loss is documented in body-composition substudies; a meaningful fraction of total weight lost is lean tissue.
- Weight regain after discontinuation is substantial — mean ~11.6 percentage points within one year of stopping in the STEP 1 extension.
- Pregnancy is contraindicated; the approximately one-week half-life means a washout of roughly two months before planned conception is advised.
- Oral formulation requires strict fasted administration; SNAC-based bioavailability is only ~0.4-1%.
Where it fits
Semaglutide is the most regulated, most studied, and most clinically deployed compound on this desk. Its evidence base spans the largest Phase 3 programs in the weight-management and cardiovascular-outcomes space.
